PASSPORT REQUIREMENTS 

        Passport services is limited to Filipinos whose legal residence is in the following states only:

New York               New Jersey             Massachusetts             Vermont             Connecticut             Oklahoma                         Louisiana                        Pennsylvania                New Hampshire           Delaware            Maine                      Rhode Island          

                A.       FIRST-TIME APPLICATION (To avoid delay, fill up application form completely & submit all requirements).

1.        Personal Appearance

2.        Original/certified copy & photocopy of Birth Certificate (from the National Statistics Office in Manila), or if not available, substitute the following (a, b, & c):

a)        Original/certified true copy of non-availability of birth certificate from the National Statistics Office (NSO)

b)       Original/certified true copy of baptismal certificate or any other acceptable public documents.

c)        Joint birth affidavit by two disinterested persons.

3.        Newly born should be registered with the Consulate (FA Form No. 40). Present child’s original birth certificate & original/photocopies of parent’s passports.

4.        Affidavit of parental consent for minors (below 18 years of age).

5.        Three identical colored photos – 1.37” x 1.77”, recently taken, front view with white background. (No eyeglasses & sleeveless attires).

6.        For military service men – in addition to items I-5, must submit certificate (with duplicate) issued by his Commanding Officer to the effect that he is a Filipino citizen.

7.        For seamen – in addition to items I-5, must submit valid Seaman’s Book.

8.        Alien Registration Card (if applicable)

9.        For lost passport applications, see item D.

B.       FIRST-TIME APPLICATION (for Lost passport application, see D below)

1.        Latest passport must be submitted for cancellation. Submit photocopy of first four pages & the page showing latest arrival of the brown passport or inside front & back covers & the page showing latest arrival of the green passport.

2.        Three identical colored photos – 1.77” x 1.37” (3.5 x 4.5 cm) recently taken, front view with white background. (No eyeglasses & sleeveless attires)

3.        For military service men – must submit certificate (with duplicate) issued by his Commanding Officer to the effect that he is a Filipino citizen.

4.        Duly filled-up (thumb marked & signed) application form. For mailed-in applications & for those not appearing in person, the application form must be notarized.

5.        For married women adopting married name – see item C on amendments below.

6.        Original & photocopy of the Alien Registration Card (for immigrants). For mailed-in applications, the Alien Registration Card must be notarized (certified true copy).

7.        Any document as proof of middle name such as birth certificate, marriage certificate, social security card, ARC, etc. Middle name should be s-p-e-l-l-e-d out (not initials). Original & photocopy must be presented.
